Montclair to College Spring 2021 Student Contract

This document addresses guidelines set forth by the MTC program. Please initial below each section indicating you have read and acknowledged your responsibilities to maintain eligibility for the MTC scholarship. -- Please sign and submit this document by Monday, December 14, 2020.

1. Sign and submit an MTC Student Contract each semester of attendance by the given deadline. *

2. Maintain continuous enrollment beginning the fall semester after high school graduation. *

3. Enroll full-time (12+ units) each fall and spring semester at Chaffey College. *

4. Apply for FAFSA or the California Dream Act each academic year. Applications must be complete, which may include additional documentation requested by the Financial Aid Office. *

5. Meet with MTC Counselor twice per semester and participate in all required (online) workshops.*

It is the student's responsibility to schedule counseling appointments online through MyChaffeyView at https://chaffey.craniumcafe.com/login. If you can't login proceed as a guest to schedule time with the MTC Counselor Desiree Jones. It is the student's responsibility to notify the MTC Ofiice if there is a time conflict well BEFORE a workshop takes place.

6. Maintain satisfactory academic progress as determined by the Chaffey College Academic Probation and Dismissal Policy (found at https://www.chaffey.edu/counseling/opening_doors/dismissal.shtml) *

Student who advance to level 1 probation will be required to attend a workshop with the MTC counselor. Students who advance to level 2 probation will no longer be eligible for MTC.

7. Right to appeal: *

In any above circumstances (except enrolling in 12 units per semester), the student shall have the right to appeal. Students may only appeal each item one time, with a maximum of two appeals within the two years of eligibility. Further instructions may be found at https://www.chaffey.edu/mtc/.

8. Update your address and telephone number on MyChaffey Portal an in the MTC Office (SL17).*

This information is used to contact you regarding upcoming events, meetings, deadlines, etc.

9. Engagement in fraudulent activity and Violation of Chaffey College Student Behavior Code. *

Students who are found cheating or committing fraud with MTC benefits/funds, or who violate the Chaffey College Student Behavior Code (found at https://www.chaffey.edu/fac-handbook/c/pdf/Student_ Behavior_CodeSH14-15.pdf), will be automatically dismissed from the program and will not be eligible for reinstatement.
