Montclair to College Spring 2021 Student Contract 


This document addresses guidelines set forth by the MTC program.  Please initial below each section indicating you have read and acknowledged your responsibilities to maintain eligibility for the MTC scholarship.  -- Please sign and submit this document by Monday, December 14, 2020.

It is the student's responsibility to schedule counseling appointments online through MyChaffeyView at https://chaffey.craniumcafe.com/login. If you can't login proceed as a guest to schedule time with the MTC Counselor Desiree Jones. It is the student's responsibility to notify the MTC Ofiice if there is a time conflict well BEFORE a workshop takes place.
Student who advance to level 1 probation will be required to attend a workshop with the MTC counselor. Students who advance to level 2 probation will no longer be eligible for MTC.
In any above circumstances (except enrolling in 12 units per semester), the student shall have the right to appeal. Students may only appeal each item one time, with a maximum of two appeals within the two years of eligibility. Further instructions may be found at https://www.chaffey.edu/mtc/.
This information is used to contact you regarding upcoming events, meetings, deadlines, etc.
Students who are found cheating or committing fraud with MTC benefits/funds, or who violate the Chaffey College Student Behavior Code (found at https://www.chaffey.edu/fac-handbook/c/pdf/Student_ Behavior_CodeSH14-15.pdf), will be automatically dismissed from the program and will not be eligible for reinstatement.
