STUDENT ID CARD REQUEST

PLEASE READ: Students requesting a Chaffey College photo ID are required to be registered for classes and cleared of all tuition fees. If you have not completed these requirements, please do so before completing this form. You will also be required to upload a photo of your government-issued identification and current photo of yourself.

REPRINTS are processed on Thursdays at no charge at the Chino, Fontana or Rancho Cucamonga Campus. Reprints may be requested on any other day with an $8.00 rush fee.

Note: An $8 reprint charge will be applied to your account. Your card will be reprinted with the photo we have on file. Any additional photo submitted will be used at our discretion, and only if we feel there is a significant change in your appearance.

Once you receive an email letting you know that your request was approved, you will have two business days to pay the fee through your Chaffey portal. If payment has not been received within two business days, your request will be removed.

Free in-person only reprints are available every Thursday for students who meet the photo ID requirements listed at the beginning of this form.

NOTE: Photo filters and inappropriate facial expressions will be rejected.

Photos must be an unobstructed, front view of the full face that is a representation of the true appearance of the card holder.
Full face, front view with a plain background. Avoid uneven shadows. Avoid glare or reflection on glasses.
No facial or hand gestures, foreign objects, or other people are to be included in the photo.
Hats, sunglasses, and any other clothing that might obstruct the view of the face may not be worn.
All headwear must be removed, unless worn for valid religious, cultural, or medical reasons.

Chaffey College reserves the right to reject any photo that does not comply with the requirements stated above.

Omnitrans

Your Student ID Card can be used as a pass for fixed route bus services. Please allow up to one week for your card to be activated by Omnitrans.
You must be actively enrolled in the term in which you use the bus services (including Summer session).
Services are available during breaks between terms if eligibility was maintained in the previous term.
Only one Student ID Card at a time will be active for bus services. Replacement cards will automatically deactivate any previous Student ID Cards issued to you.
